The medial quadriceps head, whose distal fibers (the "VMO") angle sharply inward and fire hardest in the last 15° of knee extension — the medial counterbalance that keeps the kneecap tracking straight.
Vastus medialis originates along the medial lip of the linea aspera and the intertrochanteric line of the femur, converging into the shared quadriceps tendon alongside the other three heads. Its distal-most fibers are visually and functionally distinct enough to have their own common name: the vastus medialis obliquus (VMO). These fibers run at a much steeper angle (commonly cited around 50-55°) than the more proximal, longitudinally-oriented vastus medialis longus fibers (commonly cited around 15-18°) — the oblique angle is what lets the VMO pull the patella medially rather than just adding to straight-line knee-extension force.
