Detail

Most balance training rehearses holding a position; perturbation training rehearses recovering from a sudden loss of it. By delivering controlled disturbances, it trains the reactive side of postural control (see postural-control) — the ankle, hip, and especially stepping strategies that determine whether a stumble becomes a fall.

Methods (lowest to highest tech):

  • Manual perturbations — a partner/coach applies controlled, unexpected nudges (taps at the shoulders, hips, or trunk) while the trainee maintains stance or gait; magnitude and direction are varied.
  • Self-generated — fast arm/leg reaches, ball catches and tosses, or pulling against a released band, which the trainee must counter.
  • Surface translations — moving platforms or slip/trip simulators that suddenly shift the base of support; the most ecologically valid for slip/trip falls.
  • Reactive stepping practice — being pushed past the limits of support so a corrective step is forced, training step speed, length, and accuracy.

Why it matters: protective-stepping quality is a strong predictor of fall vs recovery (see postural-control). Perturbation-based (reactive) balance training has accumulated strong evidence for reducing falls, and it targets the exact moment of balance loss that static holds do not.

Safety: perturbations must be graded and delivered with the trainee within reach of support (harness, rails, spotter), beginning with small, predictable disturbances. It is an advanced step in the progression (see balance-training-progression) and especially valuable in fall-prevention programs for older adults (see fall-prevention-overview).
