Detail

The leg extension is an excellent exercise (and, contrary to popular belief, it is not for "cutting up"/definition). Because the knee joint is a hinge joint, it is important to ensure:

i) Proper alignment of the patella with the axis of rotation of the cam. ii) Proper alignment by a slight inward rotation of the quads (hip rotation) so that extension is performed in a vertical plane.

When the lower back is pressed against the back pad, it is difficult to fully extend the knees without developing hamstring tension. Locking out is not very dangerous in this exercise because the resistance force is pushing the shins down.

Regarding working the inner/outer quadriceps safely: whichever portion is stronger will pull on the kneecap, so isolated inner/outer training is questioned on safety grounds (align the legs in the same plane; breathing is important).
